The Work Activity Report will be required for some SSDI applications, when the applicant is working or has worked since the disability began. It is used to determined if there are work incentives that can be applied to reduce countable income. This form can be completed and submitted through your MySSA Account

To complete this form, you will need:

• Paystubs, or information about your earnings
• Employer information
• Information about any job accommodations and support you received at work
