You can start the application online. You can also apply over the phone. Start by calling the national hotline at 1-800-772-1213 or by calling your local field office; they will schedule an appointment for you to start the application. If you live in Washington or Oregon, local field office numbers are no longer publicly available. You can find them organized by county here.

Once you start the online application, you have 60 days to complete it.

You will need the following to complete the application:

• Birth Certificate
• Social Security Number
• Names, addresses, and phone numbers for all medical providers
• Information about any medications you take
• Information about any jobs you have had in the last 5 years
• Proof of income
• Bank statements for any accounts that have your name on them
• Lease or mortgage
• Information about household costs

Tip: Social Security recommends applying online, and so do we. You can leave the application and come back to it with a re-entry code. We recommend doing as much online as you can.
